If you are a new San Diego State student-athlete (freshman or transfer student) or are a returning student-athlete, you are required to fill out one of the Medical Packets that are linked below. You are also encouraged to read the informational handouts below.

Please return your completed and signed medical packet to the Athletic Training Department before August 1.
